Meet our Doctors

Doctors Jacobson and Chubak were able to participate together in two Dental Outreaches with NYU. They went together to Machias Maine, and Manta Ecuador. There they treated the truly underserved patients who have no local dentists, and were faculty supervising dental students. During the outreach to Machias Maine, the team performed over 100 root canal treatments; and in Ecuador the team performed over 120 treatments. Both doctors plan to continue to participate with the NYU College of Dentistry Global outreach program.

Dr. David P. Jacobson

Dr. David Jacobson was born and raised in Bergen County New Jersey. He attended Syracuse University where he graduated with a BS in Information Technology and Management. After building an IT Consulting and Software Development Company he returned to New York University College of Dentistry where he graduated with a DDS degree. During his time at NYUCD he participated in many extra-curricular activities, was a founding member of the Peer Review Board, and was selected to the prestigous Honors in Endodontics program offered to a small group of highly motivated students with a high level of academic achievement who are particulary interested in endodontics. Dr. Jacobson was also recognized by the AAE by being chosen for the annual Student Achievement Award.

After graduating NYU with his DDS degree, Dr. Jacobson was able to go directly into the Advanced Education in Endodontics Program at NYU. During his time in this two year program he trained under internationally known faculty including Dr. Paul Rosenberg and Dr. Asgeir Sigurdsson. As a resident Dr. Jacobson participated in two research projects and was selected at the 2014 – AAE Annual Session 2nd Place Table Clinic.

Dr. Jacobson is married and has one lovely daughther. In his free time he enjoys spending time with his family, exercising, and going to the movies!

Dr. David P. Jacobson

Joshua Chubak is a native New Yorker. Dr. Chubak received his DDS degree from New York University College of Dentistry (NYU). Afterwards, he completed a one year residency at the Lutheran Medical Center Brooklyn Heights Campus Hospital – training in all aspects of dentistry and performing numerous implant and complex surgeries. Joshua then went on to complete his Advanced Specialty Training in Endodontics at NYU. He is now an American Board Eligible Endodontist.

Joshua currently lives on the Upper East Side with his wife and baby daughter.

Dr. Duong

Dr. Duong was born and raised in Brooklyn, NY. Dr. Duong received his Doctor of Dental Surgery from NYU’s College of Dentistry, with honors in aesthetic dentistry. After graduation, Dr. Duong completed a one year general practice residency at NYU-Lutheran Medical Center. It was during this time that he developed a keen interest in endodontics. However, he decided to practice general dentistry for two years to develop himself as a more well-rounded dentist before returning to NYU to specialize in endodontics.

He is an active member of the American Dental Association, New York State Dental Association, and a specialist member of the American Association of Endodontists. He is currently board eligible by the American Board of Endodontics and holds a position at NYU’s Department of Endodontics as an adjunct clinical instructor.

Dr. Duong will answer your questions and walk you through the process of a root canal procedure so that you are well-informed and can make the right decision towards your treatment.

Dr. David P. Jacobson

Dr. Forghani completed his education in Endodontics at the NYU College of Dentistry with extensive training on the use of advanced Endodontic techniques in surgical and nonsurgical procedures.

Dr. Forghani is originally from Southern California and moved to New York to receive his Doctorate in Dental Surgery from NYU and graduated with Honors. He currently serves as Clinical Associate Professor in the Department of Postgraduate Endodontics at NYU and travels periodically with the global outreach program providing dental care to underserved populations in Grenada, Mexico, Maine, and Nepal.

Dr. Forghani’s detailed care, compassion, and ability to put patients at ease by focusing on a painless root canal procedure remains the most impressive aspect of his practice.
